NDIS Support Coordination

NDIS (National Disability Insurance Scheme) Support Coordination is designed to help people with disabilities understand and implement their NDIS plans. At WesternCare Enterprise our Support coordinators will assist individuals in navigating the NDIS, connecting them to service providers, and ensuring they are making the most of their funding. There are three levels of Support Coordination, each offering different levels of support based on an individual's needs.

The Support Coordinators at WesternCare Enterprise are dedicated to assisting you in reaching your goals, enhancing your skills, and connecting you with services tailored to your needs. You’ll benefit from:

Level 1: Support Connection

This level of support coordination is designed for participants who need assistance with the basic understanding and implementation of their NDIS plan.

  • Key Focus: Connecting the participant to services and supports.

  • What it Involves:

    • Help with understanding the NDIS plan.

    • Assisting in connecting with service providers and supports.

    • Providing information about local services and community supports.

    • Ensuring the participant has access to the appropriate services.

    • Limited to basic guidance in managing the plan, with the participant expected to take a more active role in managing services.

Level 2: Coordination of Supports

Level 2 support coordination is for participants who may need more assistance with managing their NDIS plan but are still relatively independent. This level helps to coordinate and monitor services to ensure the participant is meeting their goals.

  • Key Focus: Coordinating and overseeing the supports.

  • What it Involves:

    • Helping to identify and coordinate a variety of supports and services.

    • Developing and reviewing service agreements with providers.

    • Assisting with resolving issues that may arise with service providers.

    • Working with the participant to monitor their progress and ensure their goals are being met.

    • A higher level of support than Level 1, with more frequent involvement in coordinating services.


Level 3: Specialist Support Coordination

Level 3 support coordination is for individuals with complex needs who require intensive support and coordination. This level is focused on those who face significant barriers to accessing services or require a high level of intervention.

  • Key Focus: Providing intensive, specialized support coordination.

  • What it Involves:

    • Working with participants who may have complex or challenging needs (e.g., multiple service providers, high-level coordination).

    • Intensive problem-solving and crisis management.

    • Developing strategies to manage and overcome complex issues that may arise with services and providers.

    • Providing advocacy and working closely with other professionals (e.g., health care providers, psychologists) to ensure a holistic approach to the participant’s needs.

    • Ensuring that the participant’s needs are met in a timely and efficient manner.
